I told him SEVERAL TIMES I ABSOLUTELY did NOT want this phone and the $100 credit if I had to pay for the data plan, even for 2 months.

I must admit, i am sorta late to the party on this thread, but better late than never!
I also had an issue in August 2007 with VZW sales personnel. I added a line of service to my account, and the sales lady at the store added vCast to the new phone plan even though i didnt want it.
She explained that adding vCast for the two free months was NOT optional, and that she HAD to add it to the account or else she would get in trouble. She also said i HAD to call 611 myself to cancel it, that there was nothing she could do about it.
Well, ok, rather than cause an even bigger scene in the store, i just took the phone and within 30 seconds of leaving the store i had 611 on the horn. Problem Solved.
However, i did contact the Pennsylvania Attorney General and let them know that VZW was basically cramming value added services onto accounts whether they were desired or not, probably in the hopes that customers would forget to cancel and VZW could make out on customers' inattention and laziness.
Apparently, VZW has enough troubles that they actually have a position called Attorneys General Liaison, whose job it is to receive complaints from respective AGs and deal with them. I know, because the AG Liaison called me to apologize.
I don't know what came of it, but i do know the sales lady who crammed my account was possibly fired, because i didnt see her there the half dozen or so times i went back for tech support or warranty service.
